The Towers Care Navigation Program

Thanks to a very generous grant from the Jewish Foundation of Greater New Haven’s Elderly Grants Initiative, The Towers has launched a program to serve older adults and their families across our community. In partnership with Holding Both Hands led by local community member, Abby Silbert, this program will help families navigating the complexities of the aging services system in a number of ways:

• Aging in place with help at home and/or home modifications
• Navigating a move from home to assisted living
• Considering long-term care
• Managing transitions from rehabilitation following a hospitalization
• Palliative care and more

Thanks to our generous funding, The Towers Care Navigation Program makes these services available regardless of one’s ability to pay. The Towers will be running Lunch & Learn sessions in the community where people can learn more about the program.
